Intensive efforts are currently being made to restructure the healthcare system. The example of the “Reallabor Lausitz” will be used to demonstrate how innovative cross-sector care can be achieved through digitization. With an increasing flood of data, a risk assessment can serve as an orientation aid for necessary prioritization decisions in the future.

The example of the “Reallabor Lausitz” will be used to demonstrate how innovative cross-sector care can be achieved through digitization. With an increasing flood of data, a risk assessment can serve as an orientation aid for necessary prioritization decisions in the future.
Digital solutions have been tried and tested for almost two decades, particularly in the care of patients with heart failure, and their effectiveness has been proven. However, this requires interaction between people and technology. This involves the various professions in the healthcare system as well as the patients themselves. In the future, self-management and the use of home monitoring and video consultation will have to close the gap between the increasing number of patients and the lack of specialists. Whether the use of prevention assistants in networks, especially for the treatment of coronary diabetics, is an effective means of doing so will be shown by the new results of an Innovation Fund project.
